How to use training feedback forms
Using the form builder within Natural HR, there is the ability to create training feedback forms which can be used to gather the thoughts of employees once they have attended a training course.
The feedback forms can be electronically published automatically out to employees once a course has been completed.
How to create a training feedback form
You will first need to open the form builder by going to:
Administration >Company >Form Builder
Then, you will then need to select 'Add' below the table to add a new form.
You will then be asked to input the information relating to your new form. As a minimum, you will need to input a name for the form, select Training feedback from the form type list and make sure that any fields marked with the * are completed.
Once this is submitted, you will be taken through to the form builder to start adding fields to the feedback form. Want help on how to build a form? Please visit our video library on forms here
The passage below will define each of the fields and field types to help you to build your form successfully:
Field display label - This is the text you wish to show on the screen on your form
Mandatory - If you choose yes the form cannot be submitted unless there is a value in the field
Field type - There are 44 options which you can choose from for the field type. These are:
- Date Field - A date picker field
- Text area - A multi-line text box (like Field description)
- Text box - A standard single line text box (like Field display label)
- File upload – A field which will allow attachments to be uploaded to this form.
- Check boxes - A multiple choice question which can be used to select more than one answer.
- Radio boxes - A list of custom choices where only one can be chosen
- Select drop down - A drop down list populated with custom values (like Field type)
- 1 to 5 - A drop down list pre-populated with numbers 1 to 5
- 1 to 10 - A drop down list pre-populated with numbers 1 to 10
- Score 1 to 10 - A drop down list populated with numbers 1 to 10, used specifically for scoring purposes
- Score 1 to 5 - A drop down list populated with numbers 1 to 5 used specifically for scoring purposes.
- Happiness scale – Input a graphic scale of three faces depicting the happiness rating of the user completing the form on a scale of 1 to 5
- Yes and No - A drop down list pre-populated with Yes and No
- Assigned employee (use once per form) - Please note this field is the field which should be used to assign a form to a specific employee and NOT other employees list. As the label highlights, you should only use this field once per form or you may experience unexpected behaviour.
- Other employees list - A dynamic drop-down list of all your current active employees - please note this is simply a listing of employees and does not do anything else
- Manager list - A dynamic drop-down list of all your current active managers
- Job roles list - A list populated from the Job roles section within the HR/Data management menu
- Job status list - A list populated from the Job status section within the HR/Data management menu
- Headcount classifications list - A list populated from the Headcount classifications section within the HR/Data management menu
- Departments list - A list populated from the Departments section within the HR/Data management menu
- Divisions list - A list populated from the Divisions section within the HR/Data management menu
- Companies list - A list populated from the Companies section within the HR/Data management menu
- Sites list - A list populated from the Sites section within the HR/Data management menu
- Regions list - A list populated from the Regions section within the HR/Data management menu
- Cost centres list - A list populated from the Cost centres section within the HR/Data management menu
- Business units list - A list populated from the Business units section within the HR/Data management menu
- Job grades list - A list populated from the Job grades section within the HR/Data management menu
- Heading - Text which can be used as a heading for your forms
- Horizontal Spacer - A horizontal dividing line which can be used to separate different sections of a form
- HTML - Any text which you wish to appear on the form like headings or additional instruction. This type supports changing the text size, bold, bullets etc
- Accordion - An element which is closed by default, but which can be opened on clicking
- Image - Add an image to the form
- Page Start - A page break, entering text into the "Values" box will create a section heading
- Page End - The end of a page break section (must be used with a Page start)
- Section Start - Separate your form into sections (defines the start of the section)
- Section End - Separate your form into sections (defines the end of the section)
- Clone Start – Separate a selection of fields between clone start and end which can be duplicated by the user completing the form.
- Clone End - Separate a selection of fields between clone start and end which can be duplicated by the user completing the form.
- 9 box grid Performance (standard) - A drop down list populated with Low, Moderate and High which is used to populate the performance element of the 9 box grid
- 9 box grid Potential(standard) - A drop down list populated with Low, Moderate and High which is used to populate the potential element of the 9 box grid
- 9 box grid Performance (custom) - A drop down list populated with your custom values for the x axis of the 9 box grid
- 9 box grid Potential(custom) - A drop down list populated with your custom values for the y axis of the 9 box grid
- Completed - A yes or no field which marks whether the form has been completed overall or not. Selecting yes in this field will lock down the form and mark it as complete indefinitely.
- Employee Completed - A yes or no field which marks whether an employee has completed this specific form or not.
When you click on "show advanced options"
Database field name - This is the name for the field in the database. This value must be unique and can only contain letters, numbers and underscore and may only start with a letter - spaces are not permitted. For example, employee_name is a valid name, but employee name is not. If you enter an invalid value, it will show an error when you add the field. Once the field is added this value cannot be changed.
Clone Field - If you select yes, this field should be placed inside of a "clone start and clone end" This specific field and other field set with the same permissions will have the ability to be cloned.
Field description - This is the text you can display as a tool tip icon on the form to help people understand how to fill in the field (the question mark in the green square)
Uneditable once completed - If you select yes, once this field has a value entered into it and the form is submitted, that field will not be able to be changed again.
Field Order - You can use this option to manually sort the order of the fields. You do have the ability to drag and drop the fields in form, doing this will update the field order also.
Field Weighting - This is where you set a weighting value of a field, please note when using this you should only use a numeric value.
Field Access - This is where you have options on who can see, update a field within a form
Once all your form fields have been added to the form, select 'Finish Building Form'.
How to enable your training feedback form
Firstly, you will need to define in the system which training feedback form you would like to use. To do this go to:
Administration> Company> Settings> Company Settings
Under the 'Training' section', you will see a field called "Training feedback form" - you will need to select your form from this list. - You will then need to submit this change at the bottom of the page.
Requesting training feedback
When adding a training record to the system (In Training>Course Library), you will notice a field entitled 'Request training feedback' - you will need to set this field to 'Yes' to publish the feedback form to employees.
