The following article refers to changes made to the Form Builder which can be found at:

When building or editing a form within Form Builder, the panel on the left hand side will now only show the minimum number of parameters required to add a field to your form. In most cases this will include the field display label, a drop-down box to choose whether the field is a Mandatory field, and a drop-down to choose the field type:
Depending on the field type chosen a further parameter may appear below 'Field Type'. For example, if the Field type chosen is a Select down, a 'Values' parameter will appear for you to enter the values which can be selected when the user completes the form:
Further (optional) parameters for the field you are adding are available by clicking on the 'Show advanced options' toggle:
